Pros: Molly’s Med Minder is a personal health record organizer
in a binder. It includes various forms for recording medical information,
an appointment calendar, medication cards, business card holder, and tabs
for customizing paperwork.
Cons: The $19.95 cost, plus shipping and handling, comes
to a total of $27.45, a large sum for a tabbed binder. There is no accompanying
online component for emergency access. All information is physically in
the binder and not backed up. There is no indication of how many of each
form is provided and there are no options to purchase additional pages.
The Bottomline: This is a useful tool for someone willing
to carry a binder with them to medical appointments. Without online integration,
this tool is very limited. It would actually be quite easy for an individual
to create their own organizer with the same information for the cost of
a few office supplies.

Vital Information
The binder includes a personal information section with contact and emergency
contact information areas. The insurance information section is a form with
blanks for relevant insurance documentation such as primary insurance company,
ID number, Group number, co-pay amounts, and secondary insurance. This section
also includes a space for pharmacy name, address, phone, and fax. There
is also a form for recording primary physician and specialist names, addresses,
telephone numbers, and nurse contacts.
Medication Monitoring
The medication list section is a very basic chart for medication name, dosage,
prescribing physician, and start date. The kit also includes four wallet-sized
portable medication cards.
Historical Data
A Personal Health History form is provided for recording current and past
health problems, surgeries, hospitalizations, and allergies. Users may also
indicate drug/alcohol use and a list of several conditions is provided to
prompt users to check those they have had in the past or are currently experiencing.
An immunization record with typical vaccinations is also provided with space
for users to indicate the date the immunization was received.
Daily Diary
The daily and notes sections are formatted to track symptoms, pain, vital
signs, and any other information worth recording to assist a physician in
understanding how a patient is feeling day-to-day.
