Zweena Health Record is a secure
online PHR. It is connected to the Microsoft Health Vault program and must
be allowed access to that information. The attributes that set this PHR
apart are the full-service input capabilities. ZHR collects medical records
and organizes them in the user’s online file. Once created, ZHR updates
and maintains the records.

Plans
The Family Plan is offered at a cost of $49.95/year. This includes four
connections between doctors and individuals for the year. This plan is recommended
for individuals and small families. Two years of medical history will be
collected. Phone and email support is included. Single connections may be
added, if necessary at a cost of $24.99/year.
The Unlimited Plan is offered at an annual cost of $199. This includes unlimited
connections between doctors and members and is recommended for large families
and individuals with medical conditions. Two years of medical history are
collected for this plan. Support is offered by phone and email.
The Diamond Plan is steeply priced at $499 annually. This includes unlimited
connections and is recommended for people who require extra support. Four
years of medical records will be obtained. Support includes unlimited access
to a Trained Zweena Consultant.
No credit card information is collected during the trial period. Steps to
setting up an account are very intuitive.

Collection of Medical Records
Trained staff contact doctors to collect paper health records, consolidates
the information, and converts the information to digital – to be stored
in the Zweena Health Record. Users are connected to the record for immediate
access at any time. Users may also select privacy settings to restrict access
to only those with whom they wish to share information.
Trial Plan
A free trial period is offered for users to see how the PHR works before
committing to a full-priced plan. The trial plan creates a single connection
so that users may see how the system works. Users must provide accurate
contact information, birth date, and last 4 digits of SSN to share the record
with a provider. Users may add providers and community in the trial, but
to connect members and providers and perform record collection, a fee-based
plan must be purchased.
During the trial, the user may complete their health records, including
personal information, insurance, emergency contacts, advance directives,
employer information, and information about the current plan, payment details,
and a link to manage the HealthVault account.
Each time a record is shared, the user may select the access level of the
person, including full access, view and modify, or view only.
To link to a healthcare provider, the personal information must be completed
and permission given to contact the selected provider either by phone or
email. The provider will receive a notice regarding the user’s wish to share
health records via Zweena and a response is expected. If no response is
received within 30 days, a reminder will be sent.
